Amendments

Researchers wishing to make an amendment to an ethically approved research project are required to complete a Request for Amendment Form

In addition, if the proposed amendment includes a change to study documentation, researchers are required to submit the following items to the Ethics Committee:

• One (1) copy of the amended documentation with tracked changes visible
• One (1) copy of the amended documentation without tracked changes visible
